Illinois Mutual offers disability insurance (DI) products you can purchase through your employer. If your employer currently does not offer DI products to you, ask them about it or ask your Illinois Mutual agent if Illinois Mutual worksite DI products - VSTD21 - can become available through your employer.
Voluntary Short Term Disability VSTD21 gives the peace of mind that a protected paycheck brings, with enough flexibility to meet anyone's needs. This short-term disability plan is available through eligible employers with at least three eligible employees; a minimum of two lives must be issued. As a voluntary plan, 100 percent of the premium must be paid by the employee.
Here are highlights of this easy and simple short-term plan:
- The employer may choose benefits to begin on: 1st Day Accident & 8th Day Sickness, 8th Day Accident & 8th Day Sickness, or 15th Day Accident & 15th Day Sickness.
- Benefits will be paid for 13 or 26 weeks, depending on what the employer chooses.
- You can choose to receive 66.66 percent of earned income, up to a maximum of $600 a week or less, whichever you choose.
- This plan includes a $10,000 Accidental Death and Dismemberment benefit for each insured employee.
- This plan includes a Waiver of Premium while on disability.
Plus, no evidence of insurability is required if the employee enrolls during the eligibility period and is working full-time on the effective date of coverage.
