It’s easy to make a change to your policy. Download our Policy Service Request Form then mail the completed form signed by the policyowner to:

Direct Deposit of Disability Benefit Payments

You can receive your disability income insurance benefit payment through direct deposit!* Direct deposit takes the hassle out of cashing a paper check and helps you get your disability benefit payments quickly and easily. *Payment of disability benefits via direct deposit is subject to approval of your claim by Illinois Mutual’s Claims Department.
